Beloved Menopause is a telehealth practice providing clinical evaluation, consultation, treatment planning, prescription services, and laboratory services related to perimenopause and menopause care, delivered by licensed healthcare providers acting within the scope of their license under Utah law.

This Website is provided for general informational and educational purposes and is not, by itself, a substitute for individualized professional medical care. Browsing or otherwise using the Website does not, on its own, establish a provider-patient relationship. A provider-patient relationship is formed only once you have completed the intake, identity-verification, and engagement procedures described in the Patient Intake Packet and related consent forms.

3. Geographic Scope and Licensure

Beloved Menopause's clinical Services are provided by practitioners licensed in the State of Utah, and clinical Services are available only to individuals who are physically located in Utah at the time services are rendered. Beloved Menopause does not hold itself out as offering clinical Services to, and does not accept as patients, individuals located outside Utah. If you are not physically located in Utah, you may still browse the Website's general educational content, but you will not be able to schedule or receive clinical Services, and any information provided to you through the Website in that circumstance is educational only and does not create a provider-patient relationship.

4. Patient Care & Operational Policies

The clinical, telehealth, prescribing, laboratory, billing, and emergency policies of Beloved Menopause, including detailed telehealth consent and patient responsibilities, are set forth in the Patient Intake Packet and related consent forms, which are provided to and executed by patients separately from this Website. By using this Website or the Services, you acknowledge that those operational and clinical policies govern your medical care, and that this Terms of Use document governs only your access to and use of the Website and the non-clinical aspects of the Services.

5. Disclaimer

General educational content made available on this Website including articles, guides, and other materials not delivered as part of an individual clinical encounter is provided for general informational purposes only. Carriers are not liable for delayed or undelivered messages.
